Who qualifies, and how to refer

Medicare home health eligibility

The patient is under the care of a physician or allowed practitioner who orders home health.
The patient needs intermittent skilled nursing, physical therapy, speech therapy, or continuing occupational therapy.
The patient is homebound: leaving home takes considerable and taxing effort.
A face-to-face encounter related to the reason for home care took place within 90 days before or 30 days after the start of care.

We also accept most Medicare Advantage plans. Call us and we will verify coverage before the first visit.

Making a referral

Call 800-462-5632 or fax the referral to 800-273-5331.
Include demographics, insurance, the ordering physician, diagnoses, and recent hospital or clinic notes.
Our intake team verifies benefits and contacts the patient to schedule the start of care.
A clinician completes the admission visit and sends the plan of care to the physician for signature.

Interim Administrator

Jennifer brings two decades of home health experience across clinical care, operations and regulatory compliance. A Bachelor of Science in Nursing graduate, she has built her career on making sure patients receive safe, high-quality care and that every visit meets the highest standards.

She specializes in quality assurance, diagnosis coding and patient care coordination. Her deep knowledge of OASIS documentation and Medicare and Medicaid requirements keeps the agency compliant, reduces delays, and helps patients move smoothly from hospital to home.
